Class Name:  Documentation

Class Description:  Proper documentation is crucial in all aspects of your practice.  It will help you to get properly reimbursed by insurance companies and is an integral aspect of providing quality care to all of your clients.

Topics to be covered include: taking a thorough history and physical, necessity of a HIPPA form, Oswestry Neck Disability forms, S.O.A.P. notes, client sign-in sheet, reevaluations and more!

You will receive a hard copy of a Client Intake Form, History and Physical Form, HIPPA Form, Oswestry Questionaire and a Neck Disbality form, all of which can be easily integrated into your practice on Monday morning!

Instructor(s) Biography:  Dr. Fronczak is a 1999 graduate of the New York Chiropractic College.  He also earned a B.S. in Biology from St. John Fisher College. Dr. Fronczak is the A.C.T. Program Coordinator at NYIM.
